Guy got a nat1000 on the loot table (we use roll20 to play) and got an item that allows the user to traverse to the fucking 9 realms of hell.     • I've been writing something like op described. When the group of adventurers gets into combat (it's fantasy), I make rolls for them and their enemies. Each one has a certain number of wounds they can take before they die. Keeps things interesting if even I don't know who's going to make it through.
